Edition 67 of The True Line has an article produced by Jim Watt on the subject. The drawings from the original Railway Engineer Magazine are reproduced and this shows that the operating handle did indeed extend to the other side of the wagon.

Thank you very much for posting those pictures and providing an explanation. It looks to be a far more professional way of producing bolt heads than my current method, I may well give it a go.

My method for 4mm is similar but I punch out little discs about 0.33 mm in diameter from 10 thou plastikard sheet using a steel wire punch in a press. Tony, I’d be very interested in knowing more about your punch. Do you have a picture and can you tell me more about how you made it? Thanks, Scott
